Social Services Coordinator

Responsibilities:

• Coordinate and oversee service programs for long term patients
• Develop and implement program plans and goals
• Assist in resolving grievances, concerns of patients and families
• Conduct assessments and develop individualized care plans
• Collaborate with community ancillary providers and agencies to enhance services
• Provide guidance and support to individuals and their families
• Maintain accurate documentation and records ```

Requirements:

```

• Skilled Nursing experience is preferred.
• Strong administrative skills, including documentation, web based Medical Records software
• Excellent interpersonal skills for relationship management with individuals, families, and community partners
