Job Title:
Temporary Program Coordinator, COVER (Community of Veterans Engaged in Recovery)
Location:
This is an in-custody and out of custody program position located both at the San Francisco County Jail in San Bruno, the San Francisco County Jail #2 in San Francisco, and the Swords to Plowshares Veterans Centers.
Schedule:
Monday through Friday, 8 am to 4 pm

Salary Range:
$34.00 - $35.00 per hour
Summary :
The Case Manager supports the COVER Program by providing direct service to in-custody and out of custody veterans with relationship-building, assessments, counseling, crisis intervention, post-release information and referrals, mental health services, substance abuse intervention, benefits advocacy, group facilitation, and other activities.
Responsibilities:
Case Management Communicates effectively with a variety of personalities and embraces the agency's principles of Cultural Humility. Coordinates with San Francisco Sherriff's Office (SFSO) daily, and provides care to veterans that is safe, timely, effective, efficient, equitable, and client centered. Adheres to laws regarding confidentiality and reporting requirements. Conducts intake and enrollment forms of in-custody veterans to gather information within 24 hours of the veteran booking. Perform Squares and DD214 military service verifications for in-custody veterans. Develops service and treatment goals to reduce recidivism including weekly check-ins, determination of court dates and status, and other benefits assessments. Provides supportive counseling and referrals for in custody veterans to include acting as a liaison with Jail Behavioral Health Services (JBHS), Pre-Trial Diversion, and other jail services. Participate in regular case conferences and check ins with COVER and SFSO staff for case status updates, case closures, referrals to community linkages and/or warm handoffs. Liaisons with San Francisco Veterans Justice Court (VJC) to attend court dates and may be required by judge to provide progress updates to the court judge or attorneys. Group Facilitation Reviews and implements weekly group curriculum for COVER pod and develops questions or assignments ahead of each session. Conducts weekly in-person group facilitation of evidence-based curriculum, e.g. Anger Management, Thinking for Change, Living in Balance, Harm Reduction, Emotional Intelligence, Reentry Solutions, Financial Literacy, Resolve to Violence, Framework to Recovery, Suicide Prevention, etc. Observe veterans to assess curriculum effectiveness, participant responsiveness and understanding, and adjust curriculum as needed. Coordinates with outside groups or individuals for guest speakers as needed. Ensure that participants sign in at every group conducted fully and accurately, securely transfer information to SFSO, and securely store records. Administrative • Collects the Military Service List from the San Francisco Sheriff's Office (SFSO) and prepares paperwork for new and returning veteran intakes. Maintains and updates Swords' web-based client management system (ECM). Manages client files for new and previously incarcerated clients ensuring they are complete and kept in a safe, secure, locked storage area. Compiles data for monthly reports showing newly arrived individuals and status of ongoing individuals, number of groups conducted, and other program activities. Responsible for reviewing Military Service List and Activity Report to compile list of veterans who enter and exit the program on a semi-annual basis for the SFSO. Documents releases, transfers and program placements for San Francisco Department of Public Health (DPH) contract reporting purposes. Maintains program participant files and online databases in an orderly and uniform manner in compliance with HIPAA, Department of Public Health (DPH), San Francisco Sheriff's Office (SFSO) and all other contractual requirements. Perform follow-ups and track all post-release program participants and their housing, income, benefits, employment, probation and parole statuses.
Desired Qualifications:
Experience:
Case management, group facilitation, and administrative support experience. Proficiency in PC and Microsoft systems: Outlook, Excel, Word, etc. Effectively use of cell phone, laptop, photocopier, scanner, and voice mail systems. Excellent customer service, organizational and time management skills. • Strong written and oral communication skills and demonstrated ability to interact well with people from diverse backgrounds. Must be able to obtain San Francisco Sheriff's Office Jail Clearance and to work in a jail in-custody environment. Experience working with an at-risk client, in custody, and/or post-release population is preferred. Familiarity with San Francisco community-based organizations and resources. Flexibility and willingness to perform multiple tasks in a work environment with changing priorities. Must adhere to all jail rules, regulations, and health requiremetns. Education Bachelor of Art (B.A.) in Behavioral Science (Social Work, Forensic Psychology, Community Studies, etc.) preferred, and/or: A minimum of two years' experience providing mental health/substance abuse and social services to low-income and at-risk populations
