Our mission is to provide financial assistance, family support and advocacy to low-income families with a critically ill child. Through connection, community, and care, Hearts Aligned ensures no family faces hardship alone. Position Overview Hearts Aligned Inc. is seeking a mission-driven Program Director to lead and expand our family-centered programs designed to support those caring for critically ill children. The Program Director will oversee program strategy, delivery, staff development, and community partnerships, ensuring every initiative meets the needs of the families we serve with empathy, excellence, and measurable impact. This is a leadership role for a compassionate and strategic thinker with deep experience in nonprofit program management, family services, and/or healthcare-related outreach. Key Responsibilities
  • Design, manage, and evaluate programs that directly serve families caring for critically ill children.
  • Collaborate with the Executive Director to develop strategic program goals, aligned with Hearts Aligned's mission and the evolving needs of the families we serve.
  • Provide leadership and day-to-day supervision of case managers, including training, mentorship, and performance management.
  • Cultivate relationships with hospitals, healthcare providers, social workers, and community partners to strengthen program reach and impact.
  • Oversee program budgets, ensuring effective resource allocation and compliance with funding requirements.
  • Lead program monitoring, data collection, and evaluation to ensure continuous improvement and transparent reporting.
  • Identify opportunities for new initiatives, partnerships, and services that fill gaps for families facing medical crises.
  • Provide case management services
  • Connect families to other complementary resources
  • Maintain case records, write clinical notes, and collaborate on program evaluation
  • Lead family support groups in North and South Counties
  • Represent Hearts Aligned Inc. in community partnerships, advocacy efforts, events, and public speaking engagements as needed Qualifications
  • Licensed Clinical Social Worker or Masters of Social Work - preferred
  • Minimum 5 years of experience in nonprofit program leadership, with a focus on family services, healthcare support, or crisis intervention
  • Deep understanding of the emotional, financial, and social challenges faced by families of critically ill children
  • Proven track record in program development, staff supervision, and partnership cultivation
  • Experience with Salesforce or other nonprofit client management system - preferred
  • Compassionate communicator with excellent interpersonal and problem-solving abilities
  • Commitment to the mission and values of Hearts Aligned Inc.: empathy, integrity, collaboration, and hope
  • Must be bilingual (Spanish)
  • Travel required throughout Santa Barbara County.
COMPENSATION
  • $80,000 - $105,000 annually DOE, full-time in Santa Barbara or Santa Maria
