Job Description

Are you an experienced healthcare or social services leader passionate about improving care for seniors, adults, and vulnerable populations? We are seeking an Integrated Health Services Program Manager to oversee day-to-day operations of a growing portfolio of health, social, and evidence-based programs including case management, caregiver support, fee-for-service programs, and new integrated healthcare initiatives. In this leadership role, you will partner closely with the Director of Integrated Health Services and cross-functional teams to support program operations, staff development, compliance, growth initiatives, and implementation of new integrated health services. This role is ideal for candidates with strong experience in case management operations, healthcare program leadership, Medicare/Medi-Cal populations, and team supervision. Key Responsibilities ?? Lead daily operations for Integrated Health Services programs including care management, community support, and caregiver services ?? Supervise, coach, train, and develop program staff while driving accountability and team performance ?? Develop and manage program plans, timelines, budgets, and operational workflows ?? Monitor KPIs, program growth, service delivery, utilization, and client outcomes ?? Oversee new client intake, referral coordination, and workflow efficiencies ?? Manage medical billing, claims coordination, and data integrity ??
Ensure compliance with:
  • HIPAA
  • Medi-Cal & Medicare standards
  • Contract requirements
  • Program regulations and audits ?
? Collaborate with internal departments, managed health plans, referral partners, and community stakeholders ?? Prepare operational and statistical reports (weekly, monthly, quarterly, annual) ?? Support quality assurance, utilization reviews, and continuous process improvement initiatives ?? Assist with proposals, renewals, reporting, outreach, and sustainability efforts
Qualifications Required:
? Bachelor's degree in Healthcare Management, Public Health, Human Services, Social Work, Business Administration, Operations Management, or related field OR 6+ years of related experience in lieu of degree ? 4+ years of progressive supervisory or leadership experience in case management or related field ? 4+ years of experience in healthcare, social services, or related field ? 3+ years working with adults, seniors, or vulnerable populations ? Experience managing programs serving Medicare and/or Medi-Cal populations ?
Experience with:
  • Budget management
  • Performance management
  • CRM systems
  • Case Management Systems
  • Electronic Health Records (EHR) ?
Proficiency in Microsoft 365 / Teams environment ? Valid California Driver's License with acceptable driving record
