The VA Liaison serves as the primary point of coordination between Chrysalis Center and the Department of Veterans Affairs (DVA) Transitional Living Program. This Position is responsible for overseeing program coordination, participant engagement oversight, referral management, and administrative compliance to ensure successful program operations and participant stability. This role focuses on program-level coordination rather than direct service provision and includes responsibility for maintaining collaborative relationships with the DVA, monitoring participant progress, ensuring documentation compliance, and supporting successful transitions into and through the Transitional Living Program. The DVA Transitional Living Liaison plays a key leadership role in maintaining program standards, supporting staff, and ensuring alignment with agency and DVA expectations. Hours may Vary based on program and operational needs.Essential Responsibilities:
Coordinates program services for participants enrolled in the DVA Transitional Living Program to ensure continuity of Care, Stability, and successful program engagement. Oversees the completion and timely updating of participant assessments, service plans and program documentation to ensure accuracy, timeliness, and compliance with program requirements. Monitors participant progress and program engagement, identifying barriers to success and coordinating appropriate responses to maintain program stability. Promotes best practice outcomes related to housing stability, independence, employment readiness and community reintegration. Develops and maintains strong collaborative relationships with clinical, housing, vocational, and social service agencies to support effective program operations and participant success. Serves as the primary liaison between Chrysalis Center and the Department of Veterans Affairs (DVA), ensuring consistent and professional communication regarding participant progress, program operations, and service coordination. Maintain regular and ongoing communication with the DVA Director , providing updates regarding participant progress, program needs, and operational concerns. Facilitates and participates in Monthly meetings with DVA representatives to provide program updates, review participant status, and coordinate services. Conducts interviews and participates in the screening and selection of potential program applicants to determine program eligibility and readiness for Transitional Living services. Oversees and ensures timely entry of referrals into the Point of Entry system, maintaining accurate and complete referral documentation. Conducts monthly chart audits to ensure documentation accuracy, regulatory compliance, and adherence to agency and funder standards. Reviews participant documentation and program activity to ensure alignment with funding requirements and agency expectations. Coordinates transportation resources when necessary to support participant program engagement and transitions. Supports the facilitation of structured program activities that promote participant independence, self-advocacy, and successful community integration. Prepares reports and summaries for internal leadership and DVA representatives regarding program progress, participant outcomes, and service needs. Attends and actively participates in required meetings, case reviews, administrative reviews, and program planning sessions. Maintains responsibility for ensuring and program operations align with all Chrysalis Center, Inc. policies, procedures, and standards. Performs additional administrative and program coordination duties as assigned to support the success and growth of the DVA Transitional Living Program. Key Competencies Demonstrates strong leadership and coordination skills within a program environment. Maintains operational knowledge of housing programs, service systems, and participant support frameworks. Demonstrates strong administrative oversight skills, including documentation review and compliance monitoring. Builds and maintains professional relationships with agency partners, including the department of veteran's affairs. Demonstrates effective problem-solving skills and sound decision making in complex situations. Maintains strong attention to detail and accountability for program accuracy. Demonstrates excellent organizational and time management skills. Communicates clearly and effectively in written and verbal formats. Demonstrates strong interpersonal skills and maintain a professional, service-oriented demeanor. Adheres strictly to confidentiality and HIPAA guidelines. Demonstrates initiative, independence, and leadership in maintaining program integrity. Bachelors degree in Human Services, Social Work, Psychology, Public Administration, or related field required. Master's degree preferred. Minimum of 3-5 years of experience in community-based services, housing programs, or program coordination roles. Demonstrated experience working with veteran populations strongly preferred. Experience coordination services across multiple agencies or systems preferred. Strong administrative and organizational experience, including oversight of documentation and reporting processes. Proficiency in Microsoft Office, Electronic Health Records, and referral management systems. Ability to manage competing priorities and maintain program compliance. Demonstrated leadership skills and ability to work independently and minimal supervision. Possess and maintain a valid Driver's License, reliable vehicle, current registration, and insurance.Similar remote jobs
