About The Role:
Provides Program Management Services to individuals with disabilities in the Options Day Habilitation Program, including orientation, assessments, development and monitoring of individualized service plans, and situational counseling. Serves as a member of the interdisciplinary team, ensuring compliance with program standards, accurate documentation, collaborative service delivery, and effective communication with internal staff and external stakeholders. Conduct orientation and initial assessment of individuals served upon program entry. Develop, implement, and regularly update individualized service plans in collaboration with the interdisciplinary team. Monitor progress toward goals and revise plans to reflect changes in consumer needs and objectives. Provide situational counseling and support to individuals served, assisting in problem-solving and emotional regulation. Document all services, reports, assessments, and other required paperwork in a timely and accurate manner. Maintain funding documentation and ensure compliance with payer requirements, billing procedures, and Medicaid guidelines. Coordinate with Direct Support Staff, Nurses, Program Managers, and other team members to ensure holistic service delivery. Participate in interdisciplinary team meetings, advisory groups, and community partner collaboration. Support intake and discharge planning processes, including communication with families and case managers. Assist in maintaining program compliance, accreditation standards, safety practices, and quality assurance initiatives. Bachelor's Degree in Social Work, Rehabilitation, Sociology, Psychology or Education is required. One (1) to three (3) years of experience in service coordination with people with disabilities required.
