Job Description
MYEP is seeking a Skilled Care Shift Facilitator to provide leadership and direct support within our Residential Program. This role is responsible for supporting adults with disabilities while providing on-shift guidance to staff and ensuring high-quality, person-centered service delivery. The Shift Facilitator leads by example, supports health and wellness needs, assists with community inclusion, and helps ensure documentation and care standards are met. This position requires flexibility, including some evenings and weekends, and regular use of agency software systems.
This position works in residential homes and throughout the community. The role requires the ability to travel locally, assist individuals physically, and work independently in an unsupervised setting. Essential Responsibilities Provide direct support to adults with disabilities Provide on-shift leadership, guidance, and coaching to staff Assist with personal care and skilled care supports Administer medications following agency protocols Monitor and report changes in health, behavior, or needs Ensure documentation is completed accurately and timely Assist with community outings, appointments, and transportation Support daily routines and skill development Communicate regularly with supervisors regarding service delivery Assist with training and onboarding of staff Maintain confidentiality and follow HIPAA guidelines Personal Care Responsibilities Bathing, showering, grooming, and oral hygiene Incontinence care and toileting routines Dressing and personal grooming assistance Transfers including manual lifts and overhead lifts Range of motion and physical support exercises Meal preparation and assistance with eating Monitoring food, water, and medication intake Mobility assistance including wheelchairs, walkers, and gait belts Supporting multiple individuals at one time Skilled Care Expectations Candidates must be willing to learn and provide supports including: Lift and transfer equipment Tracheostomy care Diabetic management Stoma care Catheter care Ostomy care Supplemental oxygen therapy G-Tubes / Feeding tubes Respiratory care Mental health suvpports Support during aggressive behaviors Minimum Qualifications Must be at least 18 years of age High school diploma or GED required Valid driver's license required Must be willing to work some evenings and weekends Ability to lift and provide hands-on physical supports Ability to work independently and problem solve Ability to complete required training Comfortable using agency software systems Preferred Qualifications Previous experience supporting individuals with disabilities Experience providing skilled or medical-related care Leadership or supervisory experience Strong communication and documentation skills Schedule Full-time Residential program operates 24/7 Must be available for some evenings and weekends Benefits MYEP offers a full benefits package including: Health, dental, and vision insurance Paid time off Life insurance Short-term and long-term disability Flexible spending plan 5% 401(k) match after one year of employment Full-time Residential program operates 24/7 Must be available for some evenings and weekends
