Job Overview:

As the Program Coordinator, you will be responsible for overseeing the development, implementation, and evaluation of individualized service plans for our clients with developmental disabilities. Working closely with a team of caring professionals, you will help create an inclusive and supportive environment that fosters personal growth and independence for the individuals we serve. Responsibilities:

• Conduct comprehensive assessments of individuals' needs, strengths, and goals.
• Collaborate with clients, their families, and support teams to develop individualized service plans (ISP) based on person-centered approaches.
• Coordinate and monitor the implementation of ISP goals and objectives.
• Supervise and support direct care staff to ensure the delivery of high-quality services.
• Maintain accurate and up-to-date documentation and records related to clients' progress and services provided.
• Advocate for clients' rights, ensuring their voices are heard and respected.
• Stay up-to-date with the latest developments in the field of developmental disabilities and incorporate best practices into our programs.

Qualifications:

• Bachelor's degree in a related field (Social Work, Psychology, Special Education, etc.).
• Previous experience working with individuals with developmental disabilities is highly desirable.
• Strong communication, leadership, and organizational skills.
• Ability to work collaboratively in a team-oriented environment.
• Empathy, patience, and a genuine commitment to improving the lives of others.
