Job Description
Position Summary The Program Manager is responsible for overseeing the delivery of Home and Community-Based Services (HCBS) to individuals with intellectual and developmental disabilities in accordance with Indiana state regulations, Medicaid Waiver requirements, and agency policies. This position ensures quality services, compliance with state guidelines, and person-centered planning to promote independence, choice, and inclusion for individuals served. Key Responsibilities Service Planning & Coordination Implement, and monitor Individualized Support Plans (ISPs) in collaboration with individuals, families, case managers, and direct support staff. Ensure person-centered planning and documentation are completed in compliance with state and federal requirements. Monitor individual outcomes, goals, and progress, adjusting plans as needed. Oversight & Supervision Provide leadership, guidance, and supervision to Direct Support Professionals (DSPs) and other program staff. Conduct staff training, coaching, and performance evaluations. Ensure adequate staffing levels to meet individual support needs. Compliance & Quality Assurance Maintain compliance with all Bureau of Developmental Disabilities Services (BDDS), Division of Disability and Rehabilitative Services (DDRS), and Medicaid waiver regulations. Monitor service delivery for adherence to HCBS standards, health and safety requirements, and agency policies. Complete required documentation accurately and within designated timeframes (e.g., monthly summaries, incident reports, monitoring notes). Advocacy & Support Advocate for the rights, choices, and dignity of individuals receiving services. Support individuals in accessing community resources, employment, and social opportunities. Build positive relationships with families, guardians, and interdisciplinary team members. Administrative Duties Participate in audits, site reviews, and state quality assurance processes. Maintain records, reports, and data related to service delivery and outcomes. Assist in developing program policies, procedures, and improvements. Qualifications Required Bachelor's degree in human services or a related field (Social Work, Psychology, Special Education, Rehabilitation, etc.). Minimum of 1 year of experience working directly with individuals with intellectual/developmental disabilities (IDD). Knowledge of Indiana Medicaid Waiver programs, HCBS settings, and person-centered planning. Strong organizational, communication, and leadership skills. Ability to work flexible hours, including evenings or weekends, as needed. Valid driver's license, reliable transportation, and ability to travel to service locations. Preferred Previous experience as a QDDP or Program Manager in Indiana. Familiarity with BDDS, DDRS, and state compliance systems (e.g., INsite, Therap, or similar documentation platforms). Experience working with case management agencies Supervisory or management experience in a human services setting.
