Job Description

Scope of Position:
CCRC is looking for a Full-Time nurse (LPN or RN) to serve as the Golden Apple Unit Coordinator. This position will work closely under the DON and will be responsible for the following: (Not an all-inclusive list) Guides the culture of the Golden Apple Supports daily memory care activities and special monthly activities Work with DON, Team Coordinator, and MDS Coordinator to coordinate Plans of Care Work with Social Service for Behavioral Management Plans Provide assistance to staff as needed on the floor daily Works on solutions to day-to-day problems with staff, residents, or other team members Promote customer service by upholding to and enforcing the customer service standards of CCRC Keeps staff informed of daily updates or changes Support and listen to staff/residents and promote teamwork with all members of the Golden Apple team to create the best atmosphere possible Review all new admission/returning residents/and those sent out of facility to appointments for completion of all work Keep DON updated on changes daily
Position Requirements:
Must have a valid, free and clear IN nurse license Minimum of twelve (12) hours of dementia specific training within three (3) months of initial employment as the director. Minimum of six (6) hours of dementia specific training annually to meet the needs or preferences, or both of cognitively impaired residents and to gain understanding of current standards of care for residents with dementia. Serve on the on-call rotation
Position Details:
Typically, M-F unless on-call , (Every 6 weeks) Time will be split between office and floor (3 days a week in office hours, 2 days on the cart) Eligible for insurance, PTO based on years of experience, 403(b) Compensation will be based on years of experience + additional compensation for being on call.
