Position Summary:
The IDD Program Specialist supports individuals with Intellectual and Developmental Disabilities (IDD) by coordinating services, monitoring care delivery, and ensuring compliance with regulatory requirements. This role focuses on person-centered planning, service coordination, advocacy, and collaboration with families, supports coordinators, and community partners to ensure individuals receive appropriate, high-quality supports aligned with their goals and needs.
Position Scope:
Responsible for coordinating, monitoring, and supporting services for individuals with IDD across home and community-based settings. This role ensures effective service delivery, compliance with Medicaid and state regulations, and alignment with person-centered service plans. The position also supports referral management, onboarding coordination, and community engagement to expand service access.
Key Responsibilities:
Coordinate services, staffing, and supports for individuals with IDD. Implement, monitor, and update Individual Service Plans (ISPs) and person-centered care plans. Ensure compliance with regulatory and organizational standards through accurate documentation, reporting, and record maintenance. Support staff onboarding, training, guidance, and performance monitoring. Collaborate with individuals, families, case managers, support coordinators, and community partners to ensure effective service delivery. Conduct community outreach, referral development, and advocate for individuals' rights and independence. Participate in team meetings, quality initiatives, audits, and provide backup program support as needed.

Confidentiality:
Maintains strict confidentiality of all individual health information, service records, and organizational data in compliance with HIPAA and applicable privacy laws.
Documentation Requirements:
Individual Service Plans (ISPs) Assessments and progress notes Daily service notes and GPS/time logs Incident reports Onboarding and compliance documentation Training and certification records
Certifications:
Completion of required BDS training modules Any state-required IDD or HCBS certifications (if applicable) CPR/First Aid (if required by program or state)
Education:
Bachelor's degree preferred in: Social Work Psychology Special Education Human Services Related field
Experience:
Minimum 2 years working with individuals with IDD Experience in case management, service coordination, or program support Experience with person-centered planning preferred Knowledge of Medicaid waiver or HCBS systems strongly preferred
Technical Skills:
Electronic health record or case management systems Documentation and reporting tools Scheduling and coordination platforms Basic data tracking and reporting tools (Excel or similar)
Languages:
Strong written and verbal communication skills in English required. Ability to communicate effectively with individuals with varying communication abilities.
Driver License:
Valid driver's license required with clean motor vehicle record (as applicable) to support travel to community and home-based settings.
Travel Requirement:
Requires local travel to individuals' homes, community settings, healthcare facilities, and partner agencies as needed. Occasional travel for training or meetings may be required.
