Northeast Arc is seeking a dedicated and compassionate Day Program Case Manager to join our team!
Job Type:
On-Site Full-Time Schedule:
Monday-Friday 8:00am-4:00pm
Pay Rate:
$24.00 Steps to Independence in Danvers is a community-focused day program designed to support people with building the necessary work and life skills to become productive members of their local communities.
Job Summary:
Provides supervision, opportunities, and skill development to persons served by the Northeast Arc in their community-based day and work supports program. Completes daily transportation runs for assigned individuals.
Major Responsibilities:
Evaluates and determines program participants' interests and develops individual skills. Provides oversight to assigned caseload by attending scheduled meetings, maintaining regular and professional contact with the participant's team, completing all required documentation, and ensuring the participant's progress in the program as determined by his or her goals. Completes all necessary assessments and required paperwork. Develops and facilitates meaningful activities for program participants within the program and community. Maintains all required documentation within required timeframes to include: Daily Data Monthly/Quarterly Progress Notes/6-month summaries in HCSIS Interest Inventories Assessments Confidential Record updates Any other required paperwork as assigned by supervisor. Serves as a role model and assists in developing interactive skills. Provides transportation for individuals to and from program as well as for community activities as assigned
Qualifications:
High School Diploma or equivalent required. Valid driver's license and reliable transportation required. Prior experience working with individuals with developmental or intellectual disabilities is preferred, but we're willing to train someone with the desire to learn! Ability to communicate in English and/or
ASL Physical Demands:
Must be able to stand/walk up to one hour uninterrupted throughout the day and out in the community, not a desk opening.
