Driver's License Full Job Description Description and Responsibilities:
The Director of Complex Care Integration is responsible for ensuring coordinated, trauma-informed, whole-person care for individuals with complex needs across the Community Services Division and the broader organization. This role focuses on reducing service fragmentation, strengthening cross-program collaboration, and improving care transitions for individuals involved in multiple systems. This position maintains a small, specialized caseload of high-risk individuals enrolled in the Community Support Program for Homeless Individuals (CSP-HI). In addition to direct service delivery, the Director serves in a systems-level role to support integration, program alignment, and effective service utilization.
Key Responsibilities include:
Maintain a small CSP-HI caseload with high-risk, high-complexity needs. Coordinate care across programs and systems, ensuring continuity and minimizing duplication of services. Facilitate cross-program case conferences and oversee transitions, including warm handoffs to reduce disengagement. Collaborate with internal teams and external partners (healthcare, behavioral health, housing, and justice systems) to integrate care. Monitor engagement and program utilization to support timely service access and financial sustainability. Build partnerships, represent the organization externally, and identify system-level barriers with recommendations for improvement.
Qualifications:
Master's degree in social work, counseling, psychology, public health, or related field required; or equivalent advanced experience in complex care settings. Minimum of five (5) years of experience working with individuals facing homelessness, behavioral health needs, substance use, or other complex challenges. Valid driver's license, reliable vehicle, and acceptable driving record, required.

Base Rate:
USD $37.00/Hr.
