Videos Duties Help Duties of this position include but are not limited to: Clinically and administrively screens all referred Veterans for admission to the program/CNH. Provides clinical/administrive intervention and case management to Veterans in the program. Conducts ongoing assessments on a monthly basis in person and as necessary by phone. Coordines/oversees trement with the community residential trement partner. Organizes and participes in the yearly CNH inspection process. Functions as the COR for the CNH program working with the contracting officials. Participes in the CNH Oversight Committee. Participes in required social work and geriric administrive meetings and trainings.

Qualificions Basic Requirements :

United Stes Citizenship:

Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Educion (CSWE). A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Grade Determinions:

Experience:

Licensure/Certificion:

Individuals assigned as social worker program coordinor must be licensed or certified the advanced practice level, and must be able to provide supervision for licensure. Demonstred Knowledge, Skills, and Abilities . In addition to the experience above, candides must demonstre all of the following

KSAs:

Knowledge of program coordinion and administrion which includes consultion, negotiion, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluing assigned program area(s). Ability to provide training, oriention, and guidance within clinical practice.

Preferred Experience:

Experience working with veterans and their families. Experience working with geriric populions. Experience with managing community provider relionships and monitoring veterans who are receiving care in other VA medical facilities.

Must be in overall good health, able to sit a desk working a computer, engage in light to modere physical activity including sitting, walking, bending, and carrying supplies.
