Transition Specialist

Overview Transition Specialist - This position oversees the post-secondary goals of students, develops, and implements individual community work experiences for students and assists with accessing post-secondary services, employment, or higher education as appropriate. The ideal candidate will have at least two years' experience in transition or providing community-based supports to individuals with disabilities and the ability to assist individuals in refining job goals, job development and job training. A good understanding of federal, state, and local programs that provide available community supports for independence. Familiarity with eligibility and applying for services is a must. Individual should be familiar with developing and implementing IEP/Transition plans. This position is part of a larger, dynamic post-secondary team and the ability to collaborate is important. It is a fast paced environment that requires the ability to manage multiple priorities, attention to detail and willingness to collaborate with others. A bachelor's degree is required.
