Job Description

Program Manager Lead a Team That Changes LivesWe're seeking a dynamic, mission-driven leader to oversee our Assertive Community Treatment (ACT) team—an evidence-based program supporting individuals with serious mental illness in the community.

As Program Manager, you'll guide a multidisciplinary team delivering high-impact, community-based care that reduces hospitalizations and helps people build stable, independent lives.

This role is ideal for a leader who thrives in fast-paced environments, values teamwork, and is passionate about improving systems of care.

What You'll DoTeam Leadership & Supervision
  • Supervise and support a multidisciplinary team (therapists, nurses, case managers, peer specialists)
  • Provide clinical and administrative oversight
  • Coach staff to manage complex cases and crisis situations
  • Foster a culture of accountability, collaboration, and resilienceProgram Operations
  • Ensure fidelity to the ACT model and best practices
  • Monitor caseloads, staffing ratios, and service delivery
  • Oversee scheduling, coverage, and daily team meetings
  • Manage program performance metrics and outcomesQuality & Compliance
  • Ensure compliance with state, federal, and payer requirements
  • Prepare for audits and maintain documentation standards
  • Implement continuous quality improvement initiativesCommunity & System Coordination
  • Collaborate with hospitals, courts, housing providers, and community partners
  • Support referrals, admissions, and transitions of care
  • Represent the program in community meetings and stakeholder groupsWhy This Role Matters

  • Independent clinical license (LCSW, LPC, LMFT, etc.)
  • 3-5+ years of experience in community mental health or ACT (or similar programs)
  • Prior supervisory or leadership experience strongly preferred
  • Strong crisis management and decision-making skills
