This role works under the supervision of a Qualified Supports Coordinator or Manager and adheres to Person-Centered Planning principles and regulatory requirements. Key Responsibilities Assist with implementing Individual Plans of Service (IPOS) Link, coordinate, and monitor services and supports Help individuals maintain entitlements (Medicaid, SSI, etc.) Support crisis planning and resolution Coordinate health and safety strategies, including discharge planning Promote community inclusion, self-advocacy, and relationship building Assist with housing, transportation, and employment resources Complete timely documentation and maintain compliance with agency and funder requirements
Qualifications Education:
Bachelor's degree in Human Services (eligible for advancement to Supports Coordinator after one year) OR Bachelor's degree in non-human services field OR Associate degree or 60 credit hours plus 2 years of experience with individuals with I/DD Strong communication, organizational, and computer skills Valid Michigan driver's license, insurance, and acceptable driving record Additional Requirements Extensive travel within Wayne County; reliable transportation required Ability to work flexible hours and maintain documentation standards Completion of job-specific training provided by CLS About Community Living Services (CLS) CLS provides person-centered supports for individuals with intellectual and developmental disabilities in Wayne County.
