Job Description

Independent Living Partners is seeking a CFSS Consultation Program Manager to lead our CFSS Consultation Program, supporting CFSS Consultation Coordinators as they assist individuals receiving CFSS services. In this role, you will help individuals make informed decisions about their care, develop comprehensive service delivery plans, and navigate CFSS program requirements with confidence This position involves leading CFSS consultation coordinators, providing program oversight, and ensuring individuals are served in a timely manner receiving care delivered with kindness. The role offers independent case management and collaborative teamwork. Key Responsibilities Accept CFSS Consultation clients and complete intake Develop and document Service Delivery Plans (SDPs) aligned with DHS requirements and approved funding authorizations Educate individuals about CFSS, including agency model and budget model, to support informed decision-making. Review and submit service delivery plans to the appropriate lead agency for approval. Review plan completeness, covered services, and provider options under both the agency and budget models with individuals served Monitor service utilization and documentation to prevent funding discrepancies or compliance issues. Collaborate with individuals served, case managers, and lead agencies to ensure services align with CFSS guidelines. Assist individuals served with plan modifications, provider changes, and model transitions as needed throughout the year. Conduct annuals & semi-annual reviews for eligible individuals as required by program guidelines. Maintain accurate documentation, respond to DHS data requests, and support quality assurance efforts. Recruit and hire CFSS Consultation Coordinators Train and onboard CFSS Consultation Coordinators Supervise CFSS Consultation Coordinators Provide program oversight to ensure CFSS program effectiveness Assign CFSS cases to CFSS coordinators Ensure CFSS consultation Coordinators provide person-centered and impactful services for individuals served General Skills Excellent communication and interpersonal skills Demonstrates great patience when working with individuals experiencing pain, physical & cognitive limitations and mental health challenges Experience with disability programs and services Experience working with case managers, care coordinators, lead agencies, and MCOs Ensures the work of the organization is effective, impactful, and evidence-based Consistently drives quality improvement initiatives Ensures goals are met on work plans in alignment with the strategic direction of the organization Develops programs in response to community needs and internal capacity Ensures program evaluation systems are created and maintained to meet the organization's strategic goals and measure impact Recruits, empowers, trains, and retains a diverse staff capable of providing culturally responsive approaches to client services Creates and maintains a strong collaborative culture across staff and teams Models behavior consistent with and supportive of the organization's mission and values Ensures all services are provided in accordance with evidence-based principles for supporting individuals with disabilities Advises leadership on process improvements as needed Performs other duties as assigned Qualifications A baccalaureate degree in a field related to human services: psychology, sociology, social work, counseling, special education or other human services fields. One year of full-time work experience as a CFSS Consultation Services Coordinator Required Training After Hiring Pass the DHS NET Study 2.0 criminal background study before providing services.
