Job Description

Core Job Expectation:
Train, supervise and support PCAs and 245D Staff Evaluate PCAs and PCAs work Develop a care plan with the recipient/responsible party Update and document PCA's duties Accept referrals for new clients and complete intakes Complete annual renewals for 245D clients Complete quarterly client visits for PCA clients Evaluate and understand the clients' needs Organize existing client folders Continuously monitor client files for compliance Help recruit new staff Attend team meetings Train staff on timesheets Audit files File Maltreatment & internal investigations reports General staffing support Complete monthly client file audits for compliance Support clients in emergency situations as needed General Skills General Duties Excellent communication and interpersonal skills Great patience for individuals suffering from pain and mental health Experience with disability Programs & Services Experience with working case managers and care coordinators Ensure that the work of the organization is effective (impactful) and evidence-based Work on consistent quality improvement of and program stability Ensure goals are met on work plans according to the strategic direction of the organization Develop programs in response to community needs and based on internal capacity Ensure that program evaluation systems are created and maintained to meet the organization's strategic goals and measure impact. Attract, empower, train, and retain a diverse staff able to provide culturally responsive approaches to serving clients Community engagement through serving on committees and task forces related to Face- to-Face programming Create and maintain a strong collaborative culture within staff and team Model behavior that is consistent with, and supportive of the mission and values of the organization Ensure all services are provided in light of the evidence-based principles for helping people with disabilities Advise leadership on process improvements when you feel they are needed Seek and find programs and resources to help clients pursue their goals and dreams Perform other duties as assigned Qualified Designated Coordinator qualifications: 1. A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; 2. An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; 3. A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older; or 4. A minimum of 50 hours of education and training related to human services and disabilities; and 5. Four years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Required training after hiring:
Pass the DHS NET study 2.0 criminal background study before providing services. Complete DC training within six months of hire
