Oakridge Homes promotes dignity and independent living for people with developmental disabilities and mental health needs. This full time position will office out of Brainerd, MN with possible work and day travel primarily in Crow Wing, Aitkin, Morrison or Itasca counties and the surrounding area. As a Designated Coordinator, you will: Perform highly responsible duties related to the development, implementation and coordination of Individual Service Plans for persons served with developmental disabilities or mental health needs Maintain, review and evaluate pertinent program data related to each individual's growth and development Ensure that services are being correctly implemented Provide training to direct care staff as necessary Oakridge Homes offers: Full Time-40 hours per week- choice of four 10 hour shifts or five 8 hour shifts per week Competitive wages Comprehensive benefit package including: 401(k) and 401(k) matching Health insurance Flexible spending account Dental insurance Life insurance Paid time off Referral program Vision insurance Long term disability insurance Telemedicine program EOE Oakridge Homes is currently looking for a Designated Coordinator who: Has a background in human services Has excellent verbal/written communication skills Has a valid MN driver's license and clean driving record Is able to pass a DHS background study Meets the qualifications in education and work experience by providing direct care services to persons with disabilities or persons aged 65 or older as follows: baccalaureate degree in a human services related field, and 1 year of full-time work associate degree in a human services related field, and 2 years of full-time work other education and experience may qualify and be considered
