Job Description

Our workshop is expanding and splitting, and we are looking for another DC to join our team. The position will focus on our individuals that are part of our Community Employment Supports Group. This position will work alongside our Vocational Department to make sure that our individuals are supported both during their working day and within the center while they receive Day Support Services after their work is complete. The candidate would benefit from being able to think outside the box for different activities for the individuals to engage in along with having a sense of humor. Job Overview All applicants must meet one of the Designated Coordinator Qualifications listed below: A person with the qualifying education and work experience identified under option 1, 2, 3, or 4, may act as the designated coordinator for a program licensed under Minnesota Statutes, 245D. Proof of qualifications will need to provided. Option 1: A baccalaureate degree in a field related to human services and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. Option 2: An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. Option 3: A diploma in a field related to human services from an accredited postsecondary institution and three years full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. Option 4: A minimum of 50 hours of education and training related to human services and disabilities AND four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses DUTIES but not limited to the following: Assist and/or complete intakes with person-centered served plan with team members. Developing and/or writing and implementation of each client's person-centered support plans, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan. Maintaining all signatures as required by 245-D licensing rules and regulations. Maintaining, updating all files as required by 245-D licensing rules and regulations. Completing all quarterly, semi annual and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan. Able to problem solve as needed and/or be creative to problem solve. Coordination and ensuring the protection from abuse and general well-being of clients. Overseeing staff training, and supervision in your program, regarded to person-centered needs and services as written in each person served in your program according to their specific plan.
