Job Description
The Designated Coordinator ("DC") position will be responsible for organization and delivery of compliant in-home services to individuals enrolled in a 245D program. The individuals served may be minors (under 18 years of age), adults, or the elderly. This position will report to the Designated Manager ("DM") and work on-site with their team and individual served in a 2:1 staffing ratio, determined by the individual's care plan. This position will work in-ratio 2-3 days per week as program needs apply.

$50,000 to $65,000 gross annual, dependent on knowledge, skills ability, and experienceAvailable Schedule :
Monday to Friday 8AM to 4PM and part of 24/7 on-call response We are hiring 2 DCs - one for an open program, and one for a new program slated to begin Late March to Early April! Job duties, including but not limited to: Provide supervision, training, and support to worksite staff, and support them by equipping them with what is needed to perform their essential functions Maintain positive working relationship with team, Designated Manager, and MN Care Services Leadership Work with individuals to overcome behavioral challenges with a person-centered approach and compassionate care Conduct monthly team meetings to ensure consistent communication and process follow-through by all staff Complete and maintain compliant logs and trackers related to the individual served Ensure site schedule is maintained and updated timely; ideally, two weeks in advance for the entire month Review staff timesheets for accuracy, as well as review and approve time-off requests in a timely manner Complete Medication Administration Report (MAR) reviews in partnership with nursing staff, and ensure medication is ordered in a timely manner Other cross-functional duties in or out of position scope, as necessary Required Education, Skills, and Abilities One of the three education requirements below, with additional training, certifications, and leadership experience considered: Four-year degree in a related field , such as but not limited to human services, social work, healthcare, nursing, or psychology Two-year degree in a related field, with two years of experience working with individuals with complex behavioral and/or mental health needs, in a residential, group home, behavioral health, or psychiatric setting Four years of experience working with individuals with complex behavioral and/or mental health needs, in a residential, group home, behavioral health, or psychiatric setting One or more years of Designated Coordinator experience required Must possess valid, unrestricted driver's license and clean driving record Background Study clearance, or ability to obtain clearance, from the MN Department of Human Services Knowledge and experience with Positive Behavior Support Previous experience writing 245D support plans helpful Excellent verbal and written communication skills Ability to maintain confidentiality via verbal and written communication Attention to detail and excellent organizational skills Ability to function well in a multi-priority environment, with good time management skills Positive, proactive approach in building rapport with coworkers, leadership, and individuals served Proficient in technology (email, internet, Microsoft Office products) In addition to competitive pay, we offer… Partial Medical Insurance Reimbursement for eligible positions Flexible Time Off Professional development and support from day one Recognition and rewards, like Employee of the Quarter or retailer discounts through LifeMart Employee Referral Program All qualified applicants will receive consideration for employment without regard to protected status under applicable laws.
