The LEAP Interventionist- Program Lead provides leadership and direct services within the Launching Emerging Adults Program (LEAP), a transition-focused mental health program embedded within Rochester Academy for Independent Living serving students ages 17-22.
ESSENTIAL RESPONSIBILITIES
Program Leadership:
Serve as the lead coordinator for the LEAP program embedded within RAIL Coordinate student intake, referral review, and program enrollment Facilitate collaboration with RAIL staff, special education administration, and community partners Maintain program structure aligned with LEAP goals of mental health stabilization and transition readiness Monitor program outcomes related to student engagement, mental health growth, and graduationMental Health Services:
Provide individualized mental health interventions and skill development Deliver billable services under MH-SPED reimbursement structures Develop Individualized Education Plans that include mental health goals aligned with student transitional needs Coordinate care with community mental health providers to ensure continuity of support after graduation Support students in developing emotional regulation, coping skills, and self-advocacyTransition & Case Management:
FacilitateRENEW IEP
mapping and student-led planning Support student goals related to education and graduation, employment readiness, independent living, and community engagement Coordinate with county services, vocational rehabilitation, housing supports, and mental health providers Case manage assigned students in collaboration with the LEAP Special Education Teacher Student Engagement & Relationship-Based Support Build strong relationships with students experiencing high adversity (average ACE score historically ~7.5) Provide consistent mentoring and support to rebuild student connection to education Facilitate life-readiness skill development in community-based environmentsProgram Implementation:
Support community-based instruction and transition learning Utilize NME-informed practices to support regulation and readiness to learn Maintain documentation for mental health billing and special education compliance Support program evaluation and continuous improvementREQUIRED QUALIFICATIONS
Minnesota School Social Worker license (PELSB) Experience providing mental health services to adolescents Experience with special education systems and IEP development Demonstrated experience supporting students with behavioral and mental health challenges Experience with program development and coordination focused on adolescentsPREFFERED QUALIFICATIONS
Experience with RENEW planning Knowledge of the Neurosequential Model in Education (NME) Experience with community-based transition programming Commitment toEquity:
