Quality Supports for Individuals with Intellectual and Developmental Disabilities since 1990 RISE Coordinator
  • Full time
  • $23 per hour Paid Weekly Monday
  • Friday 8am-4:00pm

The RISE Coordinator will be at least 18 years of age, have, at minimum, a high school diploma (or equivalent) and 2 years of experience within the field of intellectual and developmental disabilities. Associate degree (or higher) in a relevant field of human services, education, social sciences or closely related field AND supervisory experience in a human service setting preferred.
Function:
The RISE Coordinator will serve as an active leader, coach and trainer for the RISE program. This role will be responsible for over-seeing day to day program flow. The RISE Coordinator will give feedback, tips and assistance to RISE Direct Support Professionals (DSP's) to ensure all clients are supported in order to obtain their best possible lives. The RISE Coordinator will be responsible for administrative duties that support the day program as well as state and federal requirements
CORE RESPONSIBLITIES
Research and develop monthly and daily calendars with a variety of meaningful activities, both on and off-site including physical, educational, arts, volunteering, and recreation. Assign client responsibilities to support staff
  • maintain 1:3 ratio.
Train new staff on Willows Way Core Values, day-to-day operations, client information, equipment, van operation, lifts, safety measures, etc. Map and establish goals with each client prior to the PCSP meeting, to assist client with identifying preferred goals for the following plan year. Finalize goals during the PCSP meeting. Complete client monthly reports. Assist with updating and maintaining current client files as mandated by Willows Way, CARF, DMH and DDRB to include releases, PCSP's, physicians orders, etc. When needed, give medication (s) as prescribed and directed, complete all medication records according to Willows Way Policy and Procedures. Monitor client's health and well-being, observe changes in emotions, mobility, physical appearance or patterns of living document if needed.
