Job Description

A Program Coach at The Open Door House is committed to growing in knowledge and understanding of the various strengths, challenges, and needs experienced by young adults who aged out of foster care. Under the supervision of the Program Director and in collaboration with the Resource Coach, the Program Coach will help guide program residents in identification of needs, exercising healthy decision-making, and implementing existing strengths/abilities to make the transition to independence. The Program Coach is encouraged to integrate creativity, program advocacy, and build authentic relationships with program residents, staff, and community stakeholders. The Program Coach is responsible for maintaining up-to-date resident contact narratives, health records, any other pertinent scheduled appointment/ meetings, and individualized care plans. This position requires the Program Coach's ability to actively communicate, coordinate care with an interdisciplinary team, and provide support to a diverse group of residents. Duties and responsibilities
  • Work alongside Program Director to triage, refer, and/or initiate screening process for young adult resident inquiries and referrals
  • Facilitate new resident intake procedure, admission, and needs assessment
  • Engage residents in coaching sessions weekly or bi-weekly to gather information, assess needs, identify resources and strategies to support the resident
  • Intentionally build relationships with new residents (during initial 90-day entry period) by scheduling structured outings in the community (e.g., icebreaker lunch, job search, school enrollment visit, establishing a bank account, etc.)
  • Develop young adult resident transitional living goal plan, evaluate resident progress bi-monthly, and facilitate Resident Team Meetings
  • Demonstrate strong communication and organizational skills with the ability to maintain documentation of resident weekly contacts and track/ monitor goal progress
  • Collaborate with Resource Coach to establish a schedule that provides additional measure of resource support and hands-on learning opportunities for the resident
  • Coordinate care and develop a shared-rotation transportation schedule to support residents transportation needs to essential appointments (i.e., job interviews, school registration, healthcare appointments)
  • Maintain effective communication (verbal and written) with interdisciplinary team and serve as the liaison between residents, providers, and others involved in coordinating care
  • Participate in program staff supervision meetings to discuss resident needs, progress, and evaluate means that are integrated to support resident goals
  • Assist RA in maintaining the grounds and upkeep of the program home(s), reporting needs, and ensuring the environment is aligned with agency objectives and standards
  • Collaborate with the Program Director and Team to support resident in successful discharge planning and transition to his/ her identified plan for independent living upon program completion
Monthly:
  • Collect program fee from each resident/ provide them with a receipt
  • Submit verification of receipts to Program Director for records of deposits by the 20th of each month
  • Attend team meetings and contribute meaningful discussion and ideas to help team foster resident transitional living plan goals / meet agency-wide objectives
Occassional:
  • May be required to respond to the program house after-hours in the event of a resident or house-related emergency, though this is not common
  • Conduct unannounced overnight visits / curfew checks - if a resident concern occurs
  • Facilitate trauma-informed crisis response, de-escalation, and a therapeutic approach to managing the environment when needed as this is a shared responsibility of all staff working with residents
Qualifications Minimum Qualifications:
    Education:
    Bachelor's Degree in human services, healthcare, social work or a related field
      Licensure:
      CPR certification and training in Trauma-Informed Care is desirable
      • Ability to provide crisis response, if needed
      • Compassionate and patient with young adults who have a history of trauma
      • Demonstrates a willingness to provide quality care and support to program residents, remain calm during resident disclosures that may be difficult to process, and strong communication skills
      Preferred Qualifications:
        Education:
        Bachelor's Degree in Social Work
        • At least 2 years of experience with at risk youth/ young adults who have experienced trauma
        • Certification in Trauma-Informed Care or Crisis Intervention
        • Experience with the the foster care population
        • Experience with case management, working with inter or multidisciplinary teams, and knowledge of community resources
        • Strong general knowledge of community resources and independent living objectives
        • Ability to communicate with individuals of diverse backgrounds Working conditions Physical and emotional stress may be incurred in this position while interacting with the residents who have a history of abuse/neglect/ trauma on a regular basis.
        Lead staff are available to help process resident disclosures that can be sensitive in nature. Staff are encouraged to maintain healthy outlets. In addition, staff have access to agency-wide professional development training. Physical requirements This position requires the employee to be able to stand and walk for reasonable periods of time. Direct reports The Program Coach will report directly to the Program Director.
        Pay:
        $25.00 - $30.00 per hour
