Job Description
This position is responsible for coordinating services that address individualized support needs and enrich the quality of life for individuals supported. As a Program Coordinator, you are responsible for serving as a positive and proactive role model while providing guidance, positive direction, and assistance to people supported and staff in accordance with program policies and procedures, state and federal regulations, and personal outcome measures. A PC is an integral part of several leadership teams and serves as the liaison between all members of an individual's team. A PC works closely with team members that may include DSP's, DSPII's, ISLP's, RS(s), guardians, DDPM's, Program Directors, CEO, other PC's, agency nurses, HR, consultants, and various other agencies, medical professionals, and community members. Responsibilities of a Program Coordinator may include but are not limited to: active engagement in home and day/work environments for people supported; development of Overall Service Plans (OSPs), written supports, objectives, and data; facilitation of OSP meetings; staff training; assessments; data summarization; quality assurance practices; medication administration; attending medical appointments; managing personal finance; assisting with benefits applications and renewal; familiarity with and consistent implementation of agency policies and procedures, employee handbook, Title 19 regulations, Home and Community Based Services (HCBS) regulations, Personal Outcome Measures (POMs), emergency procedures, ANME protocols, and confidentiality protocols; Schedule will include working primarily weekday hours between 8am-5pm, but also requires the ability to be available for assistance outside of these hours to include early mornings, evenings, weekends, and night hours when the need arises. Responsibilities Provide supports to people by assisting in the development and implementation of all support plans, assist people in achieving their personal goals and desires by encouraging self-determination and active support by direct support staff. Ensure each person supported has a meaningful day to include making daily choices and being as independent as possible in their daily lives. Assist with information gathering for Personal Outcome Measures, and other assessments. Assist people with maintaining a clean, safe, and orderly home by following and teaching safety, infection control, and cleaning procedures per agency policy. Assist and is knowledgeable of emergency procedures outlined for people supported and our Agency. Maintains and expands skills and knowledge relevant to providing high quality support and services; Obtains and maintains all required certifications and trainings and applies the information to support individuals. Maintain open communication and cooperative work effort with other staff to assure quality and continuity of support to individuals served, along with participating in a team approach model for problem solving. Respect and maintain confidentiality at all times, including when working across Enable, Inc. environments. Request assistance and direction when faced with unfamiliar or unusual circumstances. Adhere to Enable, Inc.'s Mission, Vision, and Core Values while empowering each person to direct their own life choices. Adhere to Enable's medication administration policy and procedures. In 24-hour programs, medications will be administered by certified staff using Enable's med passer/med checker protocol. Report and/or document routine and unusual events including utilization of THERAP and completing daily T-logs, GER's, Med Variances, and seizure reports etc. in a concise, understandable, and legible manner. Assist people with fiscal management while safeguarding their funds. This may include completion of annual guardianship accounting reports, monitoring asset limits, developing budgets, uploading and approving receipts, bill payment, assisting people with spending choices, and communication with employers, Social Security, landlords, and other agencies. Support people to exercise their rights while training about responsibilities associated with those rights. Ensure that rights are not restricted without due process and people are treated with dignity and respect. Accept additional assignments requested when needs arise, including working at locations other than primary location(s). Serve as a role model and leader to all staff, provides support and guidance to all co-workers. Perform other duties as assigned. A four-year degree in a related field. A minimum of 2 years direct experience in the field of DD and IIDis preferred Serve as a role model and leader to all staff by demonstrating core values of respect to all people supported, their families, direct support staff, co-workers, supervisors, other agencies and community members. Valid driver's license and good driving record as defined by Enable, Inc.'s policy. Some positions may require access to a vehicle in sound operating condition and proof of current auto insurance. Achieve Certification through MSU according to State and Enable, Inc.'s policies. Ability and willingness to successfully complete and apply all training provided. Additional certifications may be required by regulations. Clear background checks as defined by regulations and policies. Ability to lift at least 50 pounds. Ability to do daily standing, bending, and lifting. Must be able to stand for 8 hours at a time. Interest in assisting people to achieve personal outcomes and person-centered supports. Must possess good verbal, reading, and written communication in English; basic math skills; and basic computer skills. Must have knowledge and skills necessary to perform ordinary household functions, cooking, cleaning, etc. Ability to work effectively and harmoniously with others, flexibility with respect to hrs. of work and changing conditions, be of good moral character, have an interest in the welfare of the individuals supported, have training and/or interest in the field of developmental disabilities, and have a willingness/ability to communicate with and relate to individuals supported, parents, guardians, fellow-staff, representatives of community agencies, and company management.
