HIT provides ample opportunities for career growth and excellent on the job training that is customized to our current programs. HIT has an opening for a Program Coordinator. This position is responsible for the overall coordination of services for those people assigned to their caseload. Program Coordinators also develop and implement Person Centered Service Plans (PCSPs) to include, but not limited to: assisting in scheduling and facilitation of the plan; writing and entering the plan; timely completion and distribution of the plan, implementation of the plan; training of the direct support staff relative to the individual plan; review and compilation of data; follow up to recommendations for services, equipment or programs, convening of the team for revision of the plan based on individual need and performance. Assures OSP as written, meets the needs/wishes of the person served and follows state guidelines. Contacts either through visits or phone calls with each person on caseload at least once a month or more depending on their level of support to evaluate satisfaction and needs are being met. Ensure the ongoing effectiveness of the OSP by observing services in action, calculation of supports and data, completion of monthly summaries of the listed outcomes in the ISP and reviewing Therap daily. EXPERIENCE
Bachelor's degree in a human service field from four-year college or university; and one year experience working with people with an intellectual or developmental disability. This position requires a six-month probationary period.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid ND Driver's License Must pursue DD Certification within 18 months (about 1 and a half years) from hire date. Must complete 5 additional modules within 6 months from hire date.
