Full Job Description Full Job Description:
The Program Coordinator is responsible for ensuring quality delivery of community-based services offered at EDS. The PC ensures these standards are met through leadership, training, coaching, and oversight of the day and residential supervisors. The PC will be under the direct supervision of the Area Director. This position will primarily be in an office setting.Duties and Responsibilities:
- Represent and Support EDS administrative decisions in a positive manner
- Focus and motivate staff and individuals receiving services to enable them to achieve EDS's mission, and 5 pillars of service.
- Maintain clear, transparent, and consistent lines of communication with Area Director, staff, persons supported, and their families
- Follow all set budgetary guidelines from Area Director
- Ensure Agency policies, procedures and all State waiver regulations are followed
- Complete regular file reviews to ensure compliance with the ISP and regulatory requirements (ISP audit) - Spot checks Quarterly. PC will pick 6 to review.
- Complete regular Book Audits to ensure compliance with Books- Spot checks Quarterly. PC will pick 6 to review.
- Service Review response / POI
- Training a proportion of Orientation class monthly
- Participate in staffing meetings and consultation with other clinicians to assess care coordination needs of individuals supported.
- Review and intake appropriate referrals to EDS services.
- Ensure team decisions are implemented as agreed upon and all goals are kept current
- Attend all ISP annual and semi-annual meetings for individuals
- Assists with the development and implementation of ISP to ensure that regulations are followed specific to right restrictions, least restrictive alternatives, approval by
RRC, ISP
approval.- Manage multiple on-going projects and other responsibilities as assigned for the success of EDS
- Adhere to all supervisor duties in the event of supervisor shortage.
- Continued coordinating of day-to-day operations to ensure day and residential services run smoothly.
- Other duties as assigned to meet the needs of the agency.
Lifting/Physical requirements:
- Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and up to 10 pounds constantly to move objects.
- Repetitive Motions
- Pushing or pulling
- Kneel, Stoop, Crouch, or Crawl (position self, move)
Qualifications/Education:
- At least 18 years old
- Bachelor's degree in human services or related field preferred
- 3 years' experience in a human service field and or 1 year's supervisory experience supporting people with developmental disabilities and/or mental health needs
- Strong attention to detail
- Ability to adapt as needed in a growing environment
- Ability to manage a variety of situations and personalities
- Nebraska Medication Aide (preferred)
- CPR/First Aid certification (preferred)
- Proof valid Driver's License and safe driving record
- Pass background checks (Central Registers for APS and CPS, National background Check and State Background Check)
- Excellent communication, interpersonal and organizational skills
- Basic math
- Excellent computer skills EDS will provide the following training: _ CPR/First Aid, Medication Aide, MANDT, Therap (documentation), Abuse, Neglect, and Exploitation, and Orientation _
