Through safe and supportive residential programs, skill-building day programs, employment support, community integration activities, and respite care for caregivers, we create an environment where everyone can thrive. Position Summary We are seeking a compassionate, detail-oriented Program Manager to oversee services for a caseload of 7-8 individuals. This role ensures person-centered planning, compliance with state and agency policies, and positive outcomes for the individuals we support. You'll collaborate closely with families, caregivers, providers, and clinical staff to build a supportive environment rooted in respect, empowerment, and inclusion. Key Responsibilities Provide program oversight for 7-8 individuals receiving services under our ABD/IDD programs. Promote an environment that protects client rights and supports personal development in alignment with their goals. Actively participate in consumer-centered planning meetings, team conferences, and interdisciplinary discussions. Conduct and document monthly site visits to monitor living environments and progress on goals. Collect and review daily notes and submit timely progress reports as required. Partner with volunteer providers to ensure timely and accurate submission of documentation including daily notes, monthly reports, and incident reports. Ensure complete and up-to-date client records, including annual documentation renewals. Prepare for and participate in mandatory state audits and site inspections; assist in resolving any compliance issues. Support the individual's health care coordination in collaboration with the company nurse. Verify that volunteer providers have completed all required documentation and Adult Foster Care training prior to working with clients. Offer support during transitions by participating in cross-training efforts when individuals are transferred between caseloads. Qualifications Bachelor's degree in Social Work, Psychology, Human Services, or a related field (or equivalent experience). Minimum 2 years of experience working with individuals with developmental disabilities, acquired brain injuries, or similar populations. Strong organizational and communication skills. Familiarity with New Hampshire DHHS regulations and person-centered planning. Valid driver's license and reliable transportation.
