Job Description
Assertive Community Treatment Program Coordinator Northern Human Services White Mountain Mental Health in Littleton NH is seeking a Program Coordinator for the Assertive Community Treatment Program. Assertive Community Treatment (ACT) is a team-based approach delivering comprehensive flexible treatment and support to individuals with the most severe and persistent mental health challenges. ACT offers individuals struggling with mental illness an intensive, community-based mental health program model that brings together a team of professionals with a holistic approach to treatment, focusing on a variety of social determinants of health including but not limited to housing, finances, medication management, natural supports, legal issues in order to increase overall well-being and independence. This position will hire, train and supervise staff, coordinate and facilitate daily team meetings, provide services to clients in their homes and in their community, provide transportation for clients as needed, monitor delivery of services to meet both state and fidelity requirements, oversees ACT referrals, discharges, monitors schedules, participate in an on-call rotation with other team members, works directly with agency QI Department to report on ACT fidelity and participate in annual state fidelity reviews.
The ideal candidate will have:
Bachelor's degree in human services, social work, sociology, psychology or similar program Prior supervisory experience Experience with Evidence Based Practices Prior experience working with individuals with mental illness Moderate computer skills including the ability to navigate MS Word, Excel, Outlook and all other agency systems including an electronic health record Must be proactive, detail oriented, able to prioritize and manage the stress of competing demands while working independently and efficientlyBENEFITS
