Department for Persons with Disabilities (DPD) is one of the largest Catholic Charities Agencies providing care for adults with intellectual and developmental disabilities in the United States. We are looking for dedicated, caring and compassionate people to work in our Group Homes. "Our agency is mandated to adhere to Executive Order regarding vaccination mandate or attaining medical/religious exemption and adhering to weekly testing". Plan Coordinator
Shift:
40 hours per week, Monday•Friday 8:45 am•4:45pm, non-exempt Some flexibility may be required to meet program needs.
Pay:
$24.20/Hr
Job Summary:
Coordinate annual, person-centered support plan with external support coordinators for each individual attending Gruenert Center to facilitate meeting and completion of plan. Work with the individual and team to identify person-centered strategies related to the goals specified in the ISP for the upcoming year, enter all strategies in Foothold fee for service software, attend all related meetings as an agency liaison and track progress throughout the year using Foothold. Provide quality assurance for Foothold notes and ensure accuracy of documentation entered by staff. This position requires a significant amount of documentation, reports, tracking, and PowerPoint presentations. The candidate must possess exceptional time management and communication skills (written/verbal), and the ability to create reports and PowerPoint presentations. Intermediate to advanced PC skills in Word, Excel and PowerPoint is required; and type a minimum of 40wpm. The employee will cooperate with the agency's administration and/or Division and DHS staff in any inspection or investigation.
Qualifications:
Bachelor's Degree and 1 year of experience working with people with intellectual and developmental disabilities: Experience in writing support plans is preferred. Valid Driver's License
