Transportation Program Coordinator (DVS #45705) State of New Mexico - 3.5 Albuquerque, NM Job Details Full-time $24.16 - $36.24 an hour 5 hours ago Qualifications Project coordination Full Job Description Posting Details Interviews to be conducted within two weeks after job posting has closed. Why does the job exist? This position will provide administrative services for the Healthcare Division under the Highly Rural Transportation and Statewide Veterans' Transportation programs. The scope of work will consist of communicating with Veterans, or their representative, to determine eligibility of program benefits, obtain transportation needs, validate appointments with third-party care facilities, document Veteran eligibility, coordinate transportation with transporters in the Veteran's county, monitor remaining balances for grant and transportation contracts, and safeguard all information collected and documented. In addition to assisting Veterans with their transportation coordination, this position will also assist Veterans with referrals to other Federal and/or State benefits offered through the NM Department of Veterans Services, or other Veteran partners, to address issues of homelessness/shelter insecurity, food insecurity, medical appointments, suicide prevention, etc. This position occasionally participates in outreach events to promote NMDVS and the transportation program. How does it get done?
Intake:
Initial point of contact to receive phone calls, texts, and emails from Veterans and/or their representative to schedule transportation to and from medical appointments covered by the Highly Rural Transportation and Statewide Veterans' Transportation programs. Veterans will be screened to determine eligibility for the programs, obtain Veterans' and appointment information, and coordinate transportation with local vendors.
Verifications:
This position will validate the appointments with the VA Hospital or applicable medical facility, will obtain necessary Veteran documentation for eligibility, and will document information in the NMDVS spreadsheet application and database system.
Coordination:
Once initial information from intake is complete, validation of appointment is confirmed, and the Veteran has been determined eligible for services, the Transportation Coordinator will coordinate with a third-party transportation contractor for the pickup and transport of the Veteran to their medical appointment. A confirmation will be relayed to the Veteran and coordination of service will be documented for grant purposes in the NMDVS spreadsheet application and database system.
Referrals:
Veteran and/or their representative may be seeking more than just transportation assistance, in which case, this position will be able to provide referrals to other NMDVS programs or other Veteran partners to provide additional services to Veterans. Who are the customers? Customers include New Mexico's Veteran population, their dependents and the NM Department of Veterans Services. Minimum Qualification Associate's Degree and two (2) years of experience program administration or project management in areas related to the purpose of the position. Any combination of education and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience. Working Conditions Work is usually performed in an office setting, with extensive exposure to Visual/Video Display Terminal (VDT), personal computer, tablet, printer, calculator, cellphone, copy machine, scanner, keyboard, and telephone usage.
