Job Description
Halo Network, Inc. is a progressive organization dedicated to providing innovative services for individuals across the lifespan, including those with developmental disabilities. The Service Coordinator Supervisor provides Supervisory support to Halo Network Inc.'s DOH Program, under the supervision of the Program Director/Chief Operating Officer. The DOH Program provides Service Coordination to individuals within the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver programs living in Long Island and New York City. The Service Coordinator Supervisor will ensure the development and implementation of service plans are based on choice and aimed at empowerment and inclusion in the community. The Service Coordinator Supervisor assumes advanced care management skills and ensures individuals receive the highest quality of services. When needed, serve as a contact for referrals, gather required documentation, and arrange for necessary screenings to proceed with intake in the applicable program. Provide participant-guided service coordination to caseload assigned, maintaining accurate and verifiable documentation of all services rendered. Using a person-centered approach, develop and complete comprehensive service plans, PPOs, and all other required documentation as set by DOH and Regional Resource Development Center. Demonstrate, promote and practice cultural competency towards participants through respect and understanding. Recognize and embrace diversity through supporting and encouraging the strengths of each individual served. Provide a minimum of 1 face-to-face contact meeting for each participant assigned monthly (meeting must occur in the participants' home on a quarterly basis), to ensure all service plans to acquire and maintain public benefits, and participants served and to monitor overall provider plans effectiveness Utilizing a team approach, maintain accessibility for clients during the workday. Assist participants in the acquisition and maintenance of public benefits e.g., Medicaid, SNAP, Social Security, etc. Provide linkage/referrals and coordination of resources/support for medical care, mental health, social, recreational and housing/basic needs, etc. Remain current in the field, support and implement best practice service models within the program. Make independent decisions and recommendations regarding immediate risk factors presented and provide crisis intervention and follow-up as needed. Demonstrate flexibility to be available to clients in their homes/communities including working outside of normal business hours as needed. Provide on-call availability via cell phone to the TBI and/or NHTD program as needed. Serve as a participant advocate. Report SRI as the incident occurs Ensure that participants' plans are individualized and strength-based Service and Detailed Plans through regular team meetings and plan reviews. department activities. Ensure compliance with all applicable local, State, and Federal regulations and agency policies. Participate in all mandatory ongoing training and professional development offered. Performs other related duties as directed by supervisor.Supervisory Core Responsibilities:
Uphold Halo Network policy and procedures. In addition, be familiar with TBI and NHTD Waiver Program Policy and Procedures Manual. Provide monthly face-to-face and case specific supervision to assigned care managers and patient navigators. MUST Complete a minimum of six (6) supervision meetings per month- utilizing Halo Network's supervision form.
INCIDENT MANAGEMENT
Report and Track all Serious Reportable and Recordable Incidence Follow up, investigate, and close the incident within the timeframe required Lead and/or participate in bi-monthly Incident Reporting Committee meeting Complete quarterly and annual SRI trend Proven management experience supervising teams within social work or behavioral health settings. Demonstrated success in program development and implementation for individuals with developmental disabilities. Strong background in relationship management with clients, families, community agencies, and funding bodies. Extensive administrative experience including budgeting, documentation, and compliance oversight. Familiarity with behavioral health practices and social work principles relevant to supporting individuals with developmental disabilities. Prior experience in supervising staff involved in program management or service coordination roles is highly desirable.
