Job Description

Rehabilitation Support Services is currently hiring a licensed clinician for our Ulster County SOS team. Safe Options Support Teams (SOS) CTI Teams are comprised of licensed clinicians, care managers and peers. Services are provided to members for up to 12 months, pre- and post-housing placement, with an intensive initial outreach and engagement period that includes multiple visits per week. Members learn self-management skills and master activities of daily living on the road to self-sufficiency and recovery. The teams' outreach facilitate connection to treatment and support services as well as providing for immediate needs. The SOS CTI Teams follow the CTI model - a time-limited, evidence-based service that helps vulnerable individuals during periods of transitions. The teams serve individuals as they transition from street homelessness to housing. CTI promotes community integration, self-advocacy, and continuity of care by ensuring that the member has strong ties to their professional and non-professional support systems during these critical periods. It is a team-based model that incorporates professionals and peers. The team works with recipients and their support networks to build skills and strengthen supports so that the SOS team can transfer care to the long-term team. SOS CTI Teams work in close collaboration with other homeless outreach providers, DSS, DCS, SPOA, hospitals and others to ensure that those individuals who are in greatest need for this intensive service are identified, referred, and immediately connected to services.
Hours:
Monday - Friday 9-5:30pm Duties include but are not limited to:
  • Community outreach and engagement where individuals experience homelessness may be living.
  • Assisting with connection to benefits and services to promote successful transition into housing
  • Completion of assessments and care plans
  • Assisting with identification and connection to informal supports in the community
  • Providing advocacy and coordination to assist with maintaining services and benefits for an assigned caseload.
  • Assisting with crisis intervention
  • Work closely with team members to provide high quality care to the individuals served
  • Provide clinical support and direction for SOS team and participants
  • Conduct outreach to community providers
  • Responsible for sharing on call duties for after hours, weekends, and holidays
Education and Experience:
A minimum of a Master's degree and possession of a current and valid professional license (LMSW, LCSW, LMHC, PhD) issued by the state of New York. Experience working with individuals with mental health, substance use, or experience working with people who are experiencing homelessness.
Other Requirements:
Valid NYS driver's license and successful clearance by Justice Center and New York State Office of Children and Families Statewide Central Register Database clearance.
