Schedule:
Monday•Friday | 8:30 AM•4:30
PM Hours:
37.5 per week

We are dedicated to enhancing the lives of individuals with developmental disabilities through innovative, person-centered programs that promote independence, inclusion, and community engagement. Position Summary CP Unlimited is seeking a full-time Habilitation Coordinator to join our team at the Long Island City Day Program. This role is responsible for supervising Habilitation Counselors and Habilitation Assistants, while designing, implementing, and monitoring individualized program plans. The Habilitation Coordinator plays a key role in ensuring high-quality services and compliance with OPWDD and NYS Justice Center regulations. Key Responsibilities Supervise Habilitation Counselors and Habilitation Assistants , including evaluations and coaching Develop, implement, and monitor individualized program plans (Life Plans) Coordinate and lead team meetings, conferences, and service reviews Ensure compliance with OPWDD and NYS Justice Center regulations Train staff on program implementation and goal achievement Participate in interviews, hiring, and staff development Maintain accurate and compliant documentation and records Act as liaison with families, residential providers, and external professionals Oversee incident management and reporting protocols Support day-to-day program operations as needed Minimum Qualifications (Required) Qualified Intellectual Disabilities Professional (QIDP) status required Candidates must meet QIDP eligibility requirements in accordance with OPWDD standards Bachelor's degree required in: Human Services Social Work Psychology Special Education or a closely related field Minimum 2 years of experience working with individuals with developmental disabilities Strong knowledge of OPWDD regulations and person-centered planning Proficiency in Microsoft Office Preferred Qualifications Supervisory experience Experience in a Day Habilitation Program Strong leadership, organizational, and communication skills
Compensation Hourly Rate:
