Position Overview:
The HSC leads the residential team to plan and deliver supports for people living in a group home setting. No two days are quite the same as the HSC interacts directly with each person on his or her caseload and coordinates the person's circle of support. The HSC must be able to lead by example to guide staff in the delivery of appropriate supports. The position offers a flexible schedule to meet the needs of the agency. Ability to travel between locations in Westchester County required.
Responsibilities:
Lead the interdisciplinary team Create staff action plans Train direct support staff to implement plans Liaison with families, day programs and care managers Ensure compliance with state and federal regulation Assist with program surveys and audits Monitor documentation of program delivery Write meeting minutes Meet monthly deadlines Ensure compliance with OPWDD program regulations
Minimum Qualifications :
Bachelor's degree in a relevant field with at least 5 years experience supporting people with autism or developmental disabilities in a residential setting is required. QIDP required. Professionalism, teamwork, great phone manner and good writing skills are required. Supervisory experience and familiarity with electronic record keeping systems is valued.
