Job Description

The Mission of Rise Above Suicide, RISE Life Services' Enrichment Experience, is to actively engage Suffolk Counties identified vulnerable at-risk youth and young adults, connect with them, and save lives by providing the highest quality of mental health services, recreational activities, family support services, educational and vocational support, and wellness strategies to enhance their lives, and instill hope for the future. While participating in the program you can expect to meet and work with a team of culturally diverse professionals each having a diverse background and a wide array of experience. Our team includes social workers, family specialists, peer specialists, outreach specialists and recreational therapists. Our team will provide participants and families support to recognize needs, identify and set goals, and create a mutual plan that will provide the best methods for success in meeting those needs and achieving your goals. RISE Life Program staff are devoted to helping you achieve your goals by providing services, which may include, but are not limited to: dance, music, art classes, daily living skills, wellness workshops, and linkage to community resources and activities. Suicide Prevention Program Team Leader
GENERAL STATEMENT OF DUTIES
Under the direct supervision of the Director of Suicide Prevention Program (Enrichment Experience), the Team Leader directs and supervises staff activities, leads team organizational and service planning meetings, provides direction to staff regarding individual cases, and who also serves as the team's Vocational & Educational Specialist. This person should have expertise in supervision of staff as well as experience in educational/vocational support, youth, and suicide prevention activities. The Team Leader will provide supportive and proactive interventions to serve all clients experiencing mental health challenges by providing directly or providing linkages to therapy, psychoeducation, care management, advocacy, and crisis intervention. The person in this role is a leader who supports individuals and their families through difficult times. Their primary role is to help improve outcomes in people's lives. It is expected that the Team Leader displays the utmost professionalism and integrity while executing the responsibilities below.
KEY ESSENTIAL FUNCTIONS
  • This staff oversees all aspects of the program
  • Conducts daily morning meetings/group supervision of the multi-disciplinary team
  • Conducts bi-weekly individual supervision of the team members
  • The Team Leader will be responsible for directing client services
  • The team leader will be responsible for the successful implementation of the program model and then managing the day-to-day operations of the team.
  • The Team Leader will serve as the point person for referrals and will be responsible for assigning and tracking outreach efforts
  • The Team Leader role will require significant multi-tasking in a fast paced, challenging environment
  • The role will require field-based work, on-call coverage, and a willingness to work flexible hours.
  • Ensures the delivery of program services in compliance with OMH regulatory standards.
  • Supervises program staff.
  • Conducts initial and continuing staff training.
  • Ensures the implementation of safety and emergency services.
  • Manages admissions and discharges, outreach and engagement, assessment and care coordination assignments.
  • Oversees assessments and care management planning to assure timely access to appropriate services.
  • Ensures the documentation of program services in client charts and program records.
  • Conducts quality assurance audits and activities to assess the effectiveness of services delivered, to ensure client charts & documentation is in compliance and to ensure that quality services are being delivered to clients.
  • Ensures required reporting to regulatory bodies and third-party payers.
  • Liaises with other providers to facilitate access to services and the coordination of care.
  • Responsible for payroll reports, reviewing employee timesheets, monitoring hours worked and vacation time.
  • Submits other reports as required.
  • Is on call for emergencies.
  • To include but not limited to other assignments/ duties as needed
EDUCATION REQUIRED
M.A. degree or higher in social work, psychology, rehabilitation counseling or a related human service field.
EXPERIENCE REQUIRED / LANGUAGE PREFERENCE
  • Minimum of 2 years' experience working with marginalized populations in a clinical capacity
  • Experience with diverse populations and age groups (children and young adults)
  • Experience facilitating group process
  • Bi-lingual is preferred
  • A minimum of 3-5 years of experience in the behavioral health field, preferably with experience on a multi-disciplinary mental health provider team.
  • Demonstrated competency in documentation of services
  • Previous management and supervisory experience.
  • Strong written and verbal communication skills and strong leadership skills.
  • Experience providing educational and vocational support to others
  • Experienced in suicide prevention strategies
  • Knowledge and experience relating to NYSOMH services and regulations
  • Current, clean drivers license required
COMPUTER SKILLS REQUIRED
:
  • Strong computer skills are required.

Pay:
$80,000.00 - $85,000.00 per year
