Full Job Description Overview:
The position has the responsibility for supervising the service delivery of Community habilitation and in-home respite. To support the mission, vision and values of Heritage Christian Services, Inc.Pay rate for this position:
$ 22.93 / hour - $ 24.23 / hour The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. SPECIFIC RESPONSIBILITIES
Be on call and act as a resource in the event of an urgent situation by working with the individual(s) to problem solve and connect with community resources/natural supports. Shall be primarily responsible for individual specific training and ongoing development of staff in the program. To supervise all program staff per assigned caseload; including completion of DSP core competency evaluations as required. To review/update individual staff action plans and safeguards at least twice annually. To assume responsibility for order and maintenance of the record keeping system, including, but not limited to: paper documentation, Therap, eVero and the agencyT:
drive/SharePoint. To coordinate and participate in staff and family meetings as needed. Provide ongoing education and support to meet the needs of employees and individuals supported. To be the primary point of contact with the care coordinator, staff members, individual/family/guardians, residential and day programs and other entities as appropriate. Ensure timely, relevant and ongoing communication with all parties to ensure continuity and quality of services. To train community support staff on documentation and compliance standards; routinely monitor documentation to ensure it meets agency quality and compliance standards. To be a good financial steward of the agency and department resources. To manage department resources with integrity (i.e. vehicle/transportation costs, staff activity fees, mileage etc.) Provide budgeting/financial support for individuals living in HCS CRO's through monitoring and communication with rep payee. To ensure accurate data collection and submission of billing for the units provided. Act as a liaison between the individuals who reside in the Customized Residential Option settings and the landlord for maintenance/facilities issues that occur (particularly after hours or when staff are not present) and submit maintenance requests accordingly. To ensure that all aspects of consumer care and business conduct are performed in compliance with our mission statement, policies and procedures, and applicable governmental laws, rules and regulations. To educate direct support professionals about the agency Compliance Plan; and to report any detected non-compliance to the Compliance Officer. Other duties as deemed necessary.Qualifications:
Required to have experience in a human service field and at least three-years' experience working with people with developmental disabilities. A Bachelor's degree is preferred; however, the minimum of a high school diploma or equivalency is required. Must show evidence of supervisory and administrative skills. A valid, New York state driver's license that is in compliance with Heritage Christian Services' motor vehicle record standards. Ability to use technology including fluency with Microsoft Office.
