JOB SUMMARY
The Rapid Transition Housing Program (RTHP) Housing Specialist assists Medicaid beneficiaries with disabilities in securing and maintaining safe and stable housing by providing supportive services and managing their rental subsidy.
Job Duties:
Perform outreach efforts to secure referrals for the Rapid Transition Housing Program and other
CDR/RCIL
services which includes developing and performing presentations to landlords and management companies, managed care organizations, hospitals, medical respite programs, homeless shelters, and skilled nursing facilities. Receive referrals, maintain a referral list, determine referral eligibility, and complete program intakes as appropriate. Identify potential housing units within the Fair Market Rent and maintain a list of available housing in the region, including the degree of accessibility, location, proximity to services, and rental cost. Assist consumers in securing stable housing, including providing assistance with addressing credit and background check concerns, transporting the consumer, helping the consumer look at housing units, meeting with the landlord, assisting with lease signings, moving, and assisting with setting up and moving into the unit. Register consumers with Section 8 and coordinate applications for other housing subsidies. Maintain consumer eligibility. Complete the annual apartment inspection to ensure that the unit complies with the program standards. Work with the consumer and landlord to address and resolve any identified issues. Secure or provide needed supports for consumers to be and remain stably housed and live independently, including rental deposit, first month rent, moving or cleaning assistance, and furniture. Perform monthly contacts with RTHP consumers to process to ensure timely rental payments and that the consumers remain stably housed. As needed, assist the RTHP Independent Living Specialist with providing information, Independent Living Skills Training, individual advocacy, and other related services to each consumer to maintain the consumer's housing, increase their independence and achieve their identified goals. Document all service delivery and program activities in both the funder's data collection system and CDR's system. Identify any issues that need to be addressed to ensure that the consumer can maintain their housing, reporting those to the Program Director.
