$19.65-$21.70

Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings. About the
Role:
We are looking for a proactive employee to perform a wide range of critical support services related to admissions and discharges for the Agency's mental health certified residential programs.
Responsibilities include:
Assist with clinical assessment and determination for admissions to mental health certified residential programs Maintain a consultation process with applicants, families, residential staff, referral sources, and admission committee in order to facilitate the decision-making process Maintain appropriate contacts with significant parties in order to assess and provide input for the Agency regarding residential services in the community Provide assistance with training new and experienced staff as circumstances dictate Maintain a person centered approach and support individual preference and choice to the extent possible under program guidelines Develop supportive relationships with individuals by learning their unique capabilities, personality, and potential Maintain appropriate records of units of service and required summaries of program services performed Provide information regarding various Agency and community programs and services to individuals and groups, i.e. applicants, their families, service providers and other persons significant to them Maintain appropriate contacts with persons in the professional community to enhance skills in providing quality program services. Facilitate positive communication with other departments and accept assignments in other areas as requested by supervisor Work collaboratively with participants, families, program staff, co-workers and supervisor Conduct self in a professional manner Attend staff meetings and other meetings as directed by supervisor Participate in in-service programs Participate in Agency-wide events, advocacy, and/or community education Provide person-centered, quality services Demonstrate effective time management skills Meet attendance and punctuality standards Observe confidentiality and safeguards all participant related information Follow Agency standards and procedures Maintain compliance with applicable training, rules, and regulations Contribute to department and Agency quality improvement initiatives Ensure compliance with federal, state, and local employment laws and regulations, as well as company policies Perform Other Duties as Assigned Education & Experience High school diploma/GED, required. Associate degree or Bachelor degree in Social Work, Human Services, or closely related field, plus 3 years' professional mental health experience, preferred. Background in performing assessments, preferred. Conditions of Employment Must have a valid NYS driver's license in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
