•Active Day has a GREAT opportunity for an ODP Program Specialist! As an ODP Program Specialist you will be responsible for understanding our members' dreams and helping enable their lives by developing their individual goals that are in alignment with their ISP.

Paid Time Off ("PTO") Medical/Vision Dental 401(k) Employee Assistance Plan Job Summary The Program Specialist provides initial, quarterly and annual assessments for adults with intellectual disabilities. The Program Specialist participates in member Individual Service Plan meetings ("ISP"). The Program Specialist is responsible for ensuring regulatory compliance through the admission process and ensures that all work complies with company policies and procedures, safety and regulatory laws and standards. Qualifications A program specialist shall have one of the following groups of qualifications: An associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities. OR A bachelor's degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities. OR A master's degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities. Minimum one (1) year experience working with adults with intellectual disabilities preferred Must be extremely detail-oriented with exceptional organizational skills in order to maintain compliance. Strong communication and relationship building skills. Ability to multi-task and manage competing priorities. Working knowledge of Microsoft Office (Outlook, Word and Excel). Successful results of background check (and successful MVR results for Hamilton OH centers only) and any other state-specific requirements. Job Responsibilities Coordinate and complete assessments and develops member goals Assess member progress with goals and objectives set forth in assessments Participate in the development of member ISPs, including any required updates/revisions Act as member "Plan" lead as applicable relating to development, annual update and revision of the ISP; and ISP review and revision Attend ISP meetings Report any discrepancies in the ISP and/or any changes related to the member's needs Ensure services provided are in accordance with ISP Review, sign and date monthly documentation of member participation and progress and make recommendations if revisions to a service or outcome are needed. Develop and implement provider services in partnership with Center Director, as required. Coordinate training of teammates in the content of health and safety needs relevant to each member. Perform all required documentation to ensure compliance with state regulations. Support all Center staff and members daily ensuring needs are met and making necessary communication efforts with support teams. Maintain a filing system; prepares case notes, reports, summaries, and documentation at the request of the Center Director. Attend meetings to address service concerns, behavioral issues and new member enrollment. Supports members as needed with personal care services, mealtime support and other direct service needs. Intervene in crises; maintains proficiency in various client techniques (i.e. AEGIS or other statespecific techniques) and represents a positive behavior that supports Active Day's culture and philosophy. Maintain required licenses and/or certificates; develops and demonstrates knowledge of progressions in field in order to provide the most beneficial member care.
