Full Job Description Responsibilities:
Designs and facilitates the Community Memory Care program, promoting active participation from residents, family members, and team members Oversees the budget and implementation of a cost-effective Memory Care program, including managing an inventory list of equipment and supplies and maximizing resources available Creates and sustains a welcoming and safe environment which promotes resident wellness and independence Participates in the resident move-in process through initial wellness assessments and the development of individualized care plans Approximately 20% of hours will be providing hands-on nursing responsibility and direct care Trains, schedules, and leads the Memory Care clinical team in providing exceptional care experiences, providing hands-on support, education and training opportunities Delivers health promotion services and dementia care education services for residents and families Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience Contributes toward quality assurance and performance improvement initiatives Coordinates the Community's infection control program while maintaining state regulatory compliance
Qualifications:
Maintains current state Registered Nurse (RN) licensure, Social Work licensure, or Dementia Care Professional certification Bachelor's degree preferred Minimum of three (3) years experience in dementia care Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members Excellent written and verbal communication skills Demonstrates exceptional interpersonal skills and ability to serve with love , compassion , and dignity
