Community Health Program Coordinator — Mercy Health Lorain Shift/Schedule Full Time
  • Scheduled for 40 Weekly Hours Primary Shift Times
  • 8am•4:30pm.
Flexibility to attend after hours and weekend events is needed. This is an on site position at Mercy Lorain Hospital Primary Function/General Purpose of Position The Community Health Program Coordinator supports the implementation of health programs within a community . This role work s closely with partners across the organization, system, and / o r community including government agencies, faith
  • based institutions, and non-profit organizations to ensure the community health program s run smoothly .
Essential Job Functions Assists with planning and implementation of community health program ( s ) in support of advancing C ommunity H ealth goals . Participates in outreach efforts, which may include engaging with local organizations, community members , and care providers to facilitate collaboration and program delivery by promoting program participation and raising awareness of health services . Coordinates operations and support related to planning, communication/promotions, logistics, and documentation of community health initiatives to reach diverse populations . May c ollect, track, and report program data to leadership to support continuous improvement and compliance . Provides input into policies and programs that improve the health of the community served including new interventions and improved delivery of health services. Education ​​​ High School Diploma or equivalent (required) ​​ Associate's or Bachelor's degree in relevant field ( preferred) Work Experience 1 years related experience (required) 3 years related experience (preferred) Language Bilingual based on market needs (preferred) Patient Population Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures. Working Conditions Periods of high stress and fluctuating workloads may occur. General office environment. May be exposed to adverse weather conditions; cold, hot, dust, wind, etc. May have periods of constant interruptions. Required to car travel to off-site locations, occasionally in adverse weather conditions. Prolonged periods of working alone.
  • Individuals in this position are required to exercise universal precautions, use personal protective equipment and devices, and learn the policies concerning infection control.
Skill s
Hard/Tech/Clinical Skills :
Proficient with computers and accuracy with data entry and Microsoft Office. Comprehensive knowledge of the field's concepts and principles. ​

Ability to learn and implement new procedures and adapt to emerging community needs. Cultural competency and ability to work with diverse populations.
