Job Description
POSITION SUMMARY
The Program Manager is responsible for the coordination, oversight, and delivery of recovery-based services on the farm. Acting as both a Peer Recovery Supporter and Case Manager to our male residents, the Program Manager provides individual guidance, connects residents to essential services, and ensures that all aspects of the recovery program run smoothly. This role is central to cultivating a supportive, accountable, and trauma-informed community grounded in recovery principles.REPORTS TO
Executive DirectorPOSITION DESCRIPTION
- Serve as a certified Peer Recovery Support Specialist, using lived experience to guide and mentor residents in recovery.
- Build trusting, non-clinical relationships with residents to promote wellness, self-advocacy, and personal accountability.
- Facilitate or co-facilitate a minimum of 2 recovery-based meetings or activities, life skills workshops, and/or peer-led groups per week independently or in collaboration with external partners to enhance resident support and community engagement
- Conduct intakes and individualized recovery support planning.
- Offer case management support to residents as needed, helping them set goals, connect with resources, and navigate challenges in their recovery journey. Work alongside each resident to ensure they feel supported and empowered throughout the process.
- Assist residents in accessing services such as outpatient treatment, medical care, mental health support, transportation, employment, and legal aid. Provide transportation to these services as needed using the company van.
- Track resident progress and maintain accurate documentation in accordance with confidentiality and ORH (Ohio Recovery Housing) guidelines..
- Conduct at least one individual recovery support session per resident per week, and a Recovery Capital Scale Assessment and Plan per resident per month, documenting interactions in a timely and accurate manner.
- Complete all required documentation within 24 hours of service delivery.
- Oversee resident compliance with daily schedules, activities, and farm-based responsibilities.
- Conduct and document urine drug screen (UDS) tests according to program protocols and maintain confidentiality at all times.
- Upload resident documents into the EHR (Electronic Health Record)
- Monitor and document resident compliance with self-administered medications, ensuring safety and accountability.
- Supervise and support Resident Leads.
- Collaborate with the Executive Director on strategic planning, policy, program development, and crisis response.
- Respond to resident concerns, conflicts, or crises with professionalism and using trauma-informed approaches.
- Uphold safety standards and contribute to a physically and emotionally secure environment.
- Coordinate discharges, aftercare planning, and referrals to other levels of care when needed.
- Model healthy boundaries, emotional regulation, and problem-solving.
- Serve as a resource to residents, staff, and others on matters relating to the program policies, procedures, and activities.
- Serve as an active member of the Program Committee, providing input on program development, resident support strategies, and continuous improvement initiatives to enhance overall service quality.
- Maintain positive relationships with external providers, courts, and referral partners.
- Represent St. Zachary's Haven at meetings, events, and in outreach efforts when needed.
- Complete required forms and documents in accordance with company policy and state and/or federal regulations.
ORH SPECIFIC DUTIES
- Assist staff in responding when a resident is not upholding the Code of Conduct
- Help monitor residents for potential warning signs concerning the recurrence of symptoms and refer them for more support.
- Assist staff with checking in with residents on a daily basis
- Meet with residents at least weekly to discuss their recovery plans
- Be available to residents if they need additional support
- Assist in orienting new residents to the home and ensure all policies and procedures are explained to them
- Ensure that resident-led house meetings happen at least weekly and that all residents attend.
- Assist the team in examining the physical property and spend enough time at the home to ensure that all residents are accounted for, residents do not need additional support, and the house is free from substance use.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
- Minimum of 2-3 years' experience in recovery services, residential support, or case management.
- Familiarity with Electronic Health Record (EHR) systems and accurate documentation practices.
- Experience conducting urine drug screens and monitoring self-administered medication is preferred.
- Strong interpersonal, organizational, and boundary-setting skills.
- Demonstrates a passion for the organization's mission, enthusiasm, and professionalism.
- Self-motivated and able to work independently.
- Knowledge of crisis intervention, multicultural and socio-economic issues, and advocacy skills.
- Comfortable working in a small team atmosphere.
- Computer skills: Microsoft Office suite of products, including Word and Excel
JOB REQUIREMENTS
- CPR certification and Narcan training required (may be completed during onboarding if not already obtained)
- Certified as a Peer Recovery Supporter (PRS), or ability to become certified within 90 days of hire
- Lived experience in recovery (minimum one year)
- High school diploma or equivalent; Associate's or Bachelor's degree in Social Work, Human Services, or related field preferred
- Valid driver's license and ability to pass background and drug screening.
SCHEDULE
- This is a full-time salaried position (40 hours per week) with a flexible schedule.
