Job Description

We are seeking caring, motivated, and dependable professionals who are passionate about helping others succeed and thrive within their communities. Schedule Full-Time, 40 hours per week Primarily Monday-Friday, 8:00 a.m. - 4:30 p.m. Schedule may vary based on program needs. Compensation Commensurate with education and experience. Position Summary The Program Specialist is responsible for coordinating daily program operations and providing direct support to Individuals receiving services. This position plays a key role in ensuring high-quality care, regulatory compliance, and person-centered programming.
Responsibilities include:
Maintains a high level of compliance and works effectively with all licensing entities including but not limited to the Office of Developmental Programs and Human Services Agencies Performs Assessments, executes and maintains Individual Support Plan (ISP), facilitates annual and quarterly ISP reviews for each Individual enrolled in the training facility. Works with family members supports coordinators, support staff and outside agencies to ensure a holistic interdisciplinary team approach to the positive progress of Individuals. Acts as an advocate for Individuals. Assembles, complies and ensure accuracy of billing. Submits billing to the appropriate person on or before the due date. Acquires and maintains all certifications relevant to the position. Assists in the planning, arrangement and implementation of special agency events and projects. Participates in and performs duties for various organizational committees. May assume the lead role on the certain committees. In conjunction with the Director, researches, prepares, and presents staff trainings on pertinent agency topics. Develops and implements daily lesson plans to develop Individuals' independent living skills, work skills and community knowledge in conjunction with goals contained in Individual Support Plans (ISP). Promotes and provides opportunities for involvement in the community in those activities which the Individual expresses interest. Assigns and/or provides daily supervision of Individuals while engaged in program activities, break/lunch and during arrivals and departures. Provides positive role modeling by working directly with Individuals engaged in the performance of tasks. Records all tasks performed, goal achievement status and related observations on a daily basis. Coordinates transportation schedules and safely transports Individuals or supervises Individuals during daily transportation when directed to meet program needs. Enforces proper vehicle etiquette for Individuals. Assists with ambulating, toileting, transferring, feeding, meal preparation, clean up and personal care as necessary. Attends to individual's personal and program needs as necessary. Reports medical issues immediately. After proper training, administers first aid as necessary. Completes all required paperwork. After proper training, administers medications in accordance with Department of Human Services (DHS) regulations for medication administration. Has or willing to gain a working knowledge of PA Codes and regulations to ensure protection of the health, safety, and well-being of Individuals with disabilities, through the implementation and enforcement of the minimum requirements for the operation of agency facilities. Qualifications Candidates must meet one of the following qualifications: Master's degree and one (1) year of experience working directly with individuals with disabilities; OR Bachelor's degree in a related field and two (2) years of experience working directly with individuals with disabilities; OR Associate's degree or 60 college credits and four (4) years of experience working directly with individuals with disabilities
Additional Requirements:
ACRE Certification (or willingness and ability to obtain) Medication Administration Certification (or ability to obtain) CPR/First Aid/AED Certification (or ability to obtain) Ability to effectively use computer systems and technology Strong written documentation and communication skills Self-motivated with the ability and desire to teach and support others Ability to complete at least 24 hours of annual training Driver Requirements Must possess a valid driver's license and maintain a satisfactory three (3) year driving record to qualify as an approved Agency driver through the Agency's insurance carrier.
