Responsibilities include a combination of program administration and direct service to Office of Behavioral Health (OBH) Individuals Served to obtain and maintain their permanent supportive housing. In conjunction with the Individuals Served and members of the interdisciplinary team, completes individualized housing assessments and plans that meet the standards of funders and accrediting bodies. Assists staff in the development of Individuals Served housing assessments and plans. Works with landlords, coordinates moves, and assists with chart audits and data submission to funders.
