This role is central to improving maternal and child health outcomes for at-risk youth and young adults through trauma-informed, relationship-based care. The Program Coordinator serves as the primary service provider and project lead, responsible for client engagement, case management, care planning, outreach, and program coordination. About the Program RPCMP provides intensive, individualized support to youth ages 10-25 who are pregnant, parenting, or at risk, with the goal of reducing adolescent pregnancy, improving birth outcomes, and breaking cycles of trauma and poverty. Key Responsibilities Direct Service & Case Management Conduct client outreach, intake, and enrollment Complete comprehensive assessments and baseline screenings Develop individualized care plans within 7 days of intake Provide ongoing case management, referrals, and service coordination Maintain consistent follow-up throughout pregnancy and postpartum Respond to client communication within 1 hour Program Implementation Deliver structured individual sessions for clients Coordinate referrals (OB/GYN care, WIC, mental health, insurance, home visiting, etc.) Support crisis intervention and safety planning, including IPV cases Collaborate closely with Program Manager, Doula, and Case Manager Work with schools, create and deliver reproductive health education presentations throughout the state. Outreach & Community Engagement Maintain active relationships with referral partners (schools, DCF, courts, healthcare providers, community agencies) Conduct ongoing outreach and recruitment activities statewide Documentation & Compliance Maintain accurate, timely documentation (within 48 hours of client contact) Track outcomes and service delivery in program database Ensure compliance with HIPAA, mandated reporting, and DPH contract requirements Qualifications Bachelor's degree in social work, human services, public health, or related field (required), master's degree (MSW) a plus.
