Position:
Service Coordinator Madison County Service Coordination is looking for a motivated, self-starter, with a passion for helping others. Duties:
Manage assigned caseload of individuals with developmental disabilities, as defined by the Missouri Department of Mental Health, Division of Developmental Disabilities. Willingness to share in the agency's mission and values by participating in agency lead, community outreach activities. Develop service plans, apply individuals for needed services and complete required oversite of approved services. Link clients and families to state and/or local resources. Complete routine documentation such as case notes, service monitoring reports, individual support plans & addendums, assessments, and other reports as needed concerning the client's services and needs, in the contractually mandated timeframes. Attend inter-disciplinary team meetings to discuss client cases in relation to eligibility, support plans, progress, and possible changes in the service program. Monitors services to ensure that the terms of the authorization are being fulfilled by the authorized provider, to ensure quality of services, and review client progress. Requests modifications for non-productive program or enrolls the client in an appropriate program fitting for their individual needs. Willing and able to drive in Madison and Bollinger Counties to meet with individuals, families and service providers. Attend staff meetings concerning new or revised policies and procedures. Visits provider agencies, attends meetings, and/or visits with clients, as directed by DMH-DD depending on their approved program(s). Attends conferences, reviews literature, and confer with other Service Coordinators to become informed concerning community resources, concerning practices in the field of IDD, special education, and health care for people with ID/DD diagnosis. Conduct client assessments as needed to determine eligibility for new and ongoing services/programs. Maintain accurate and current client files (electronically) Complete on-going training, as needed, to maintain compliance with policies and procedures.
Requirements:
Bachelor's degree in a human services field. Able to pass background screenings. Able to pass a drug test. Valid driver's license and dependable transportation.
