Support Coordination Job Description Job Summary Join our dynamic team as a Support Coordinator, where you will play a vital role in empowering individuals to achieve their personal goals and enhance their quality of life. In this energetic and rewarding position, you will coordinate services, facilitate access to resources, and provide compassionate guidance to clients facing diverse challenges. Your proactive approach will ensure seamless support delivery, fostering independence and well-being for those in need. This paid position offers an exciting opportunity to make a meaningful difference in the community while developing your skills in behavioral health, social work, and crisis management. Responsibilities Develop personalized support plans tailored to each client's unique needs and goals Coordinate services across multiple agencies, including healthcare providers, social services, and community organizations Assist clients with navigating complex systems such as Medicaid, Section 8 housing, child welfare programs, and workforce development initiatives Provide crisis intervention and crisis management support during urgent situations to ensure client safety and stability Utilize motivational interviewing techniques to encourage positive behavioral changes and foster client engagement Maintain detailed documentation of client interactions, service plans, and progress notes in compliance with organizational standards Qualifications Proven experience in social work, behavioral health, or related fields with a focus on client advocacy and support coordination Strong skills in crisis intervention and crisis management to handle urgent situations effectively Working knowledge with OCDD Medicaid Waivers and OAAS Targeted Populations Experience supervising staff or mentoring peers is preferred Excellent communication skills with the ability to motivate and empower clients through motivational interviewing techniques Join us in making a positive impact! Pay:
$18.50 - $21.50 per hour
