This position requires a compassionate and organized professional who is adept at managing relationships, overseeing program development, and ensuring that all administrative tasks are handled efficiently. The ideal candidate will possess a strong background in social work and behavioral health, along with experience in supervising teams and managing program budgets. Responsibilities Coordinate the transition process for individuals moving into new programs or services. Develop and implement effective transition plans tailored to individual needs. Supervise staff involved in the transition process, providing guidance and support as needed. Manage program development efforts to ensure alignment with best practices and regulatory requirements. Oversee budgeting processes related to transition programs, ensuring financial accountability. Maintain strong relationships with stakeholders, including families, service providers, and community organizations. Conduct assessments to identify the needs of individuals transitioning into new services. Facilitate training sessions for staff on best practices in supporting individuals with developmental disabilities. Ensure compliance with all relevant policies and regulations throughout the transition process. Requirements Bachelor's degree in Social Work, Psychology, or a related field Proven experience in supervising teams and managing programs within the behavioral health or social services sectors. Strong skills in program development and budgeting. Excellent relationship management abilities to effectively engage with individuals, families, and community partners. Knowledge of best practices for working with people with developmental disabilities. Exceptional organizational skills and attention to detail. Ability to manage multiple priorities in a fast-paced environment. Strong communication skills, both verbal and written.
